Transcript Ordering & Replacement Diplomas

The University of St. Augustine for Health Sciences has retained Parchment to accept and process transcript orders on the internet. You are able to have official transcripts sent electronically or by mail.  We always recommend contacting the recipient before placing an order to confirm the preferred delivery format.

Program Readmission Request Form

The Readmission form is only for students who withdrew from the program or were withdrawn due to the continuous enrollment policy.  The student's last day of attendance must be within 1 year.  If longer than 1 year the student needs to apply through admissions.
